If Bank 1 is proving unsuccesful boot bank 0 and try eurasia to flash with.
make sure the chip is not a clone. Make sure your bios file is labeled bios.bin

QUOTE
Hey, on that guide it says that the DuoX2 needs bioses that are 256K, it should be 512K.

The information is referring to flashing via evox because evox automatically resizes the bios file. So with evox 256k can indeed be used.
Using a bios disc the size has to be 512k as it does with eurasia and other flash apps that do not resize the bios

Make sure the flash IDs for the chip are in the first 13 lines of the raincoat.cfg file.

Flash = 0xbf52,"Spiderchip",0x40000
Flash = 0xbf10,"SST 29EE020",0x40000
Flash = 0xbf12,"SST 29LE020/29VE020",0x40000
Flash = 0xbfd6,"SST 39LF020/39VF020",0x40000

Flash = 0xbf61,"SST 49LF020",0x40000
Flash = 0xbf52,"SST 49LF020A",0x40000
flash = 0x01d5,"AMD - Am29F080B",0x100000;

flash = 0x01a4,"AMD - Am29F040B",0x80000;
flash = 0xda0b,"Winbond - W49F002U",0x40000;
flash = 0xc236,"MACRONIX - MX29F022NTPC",0x40000;
flash = 0x20b0,"ST M29f002BT",0x40000;
flash = 0x01da,"AMD - Am29LV800B",0x100000;
flash = 0x04a4,"FUJITSU - MBM29F040C",0x80000;
flash = 0xadb0,"Hynix - HY29F002TT-90",0x40000;

Copy and paste this into notepad. save as raincoat.cfg  The file extension must be cfg
this goes on the bios disc not on the flash disc. So it goes with your bios.bin file.

Chnacer is correct but none of this is needed ! i have prepared everything for you !

All you need to do is either grab

Flash Disc for xbox v1.0 - V1.5 or
Flash Disc 16 for Xbox V1.6

and burn eurasia to a cdrw and the correct flash disc for your console to another cdrw and no editing is needed and it's all in the link i sent you before ...

If you can't follow my guide then you will have no hope flashing your chip as it dont get any easier !

Here, I just went ahead and did it.

UNKNOWN DEVICE Read Only???  manf=0x09, dev=0x00
Try adding the device ID to  raincoat.cfg

And it lists the codes that I put in the raincoat.cfg from Chancer's post above that, I put in the ones from the first mention of AMD, down since my chip says AMD on the top.

The raincoat.cfg does not go on the flashdisc but on the bios disc. It would seem your chip is not listed in the cfg file built in to the eurasia flashdisc so it tells yuo to insert the bios disc. when you put in the bios disc it looks then for the device id in the raincoat.cfg file on the bios disc.
The exact wording may help in this case.
Do you put in the bios disc when the drawer opens after the "Flashing will continue" message?

So it loads the eurasia flash disc ok but fails to load the actual bios disc? Even the one with the raincoat.cfg on it?
If it still does not accept the flash from the bios disc with the raincoat file on as well as the bios then the problem is either in the way you are making the bios disc or the chip could be a clone with a flashrom thats not recognised

So your bios disc has bios.bin (your 512K hacked Bios) raincoat.cfg (the list of flash iDs) and a dummy file and you burnaed this as  a regular data disc.
If this is a yes to all the above then check the chip carefully because the only whites that were giving the problem have all turned out to be clones. They are usually quite obvious though so its unlikely. (Silver contacts, poor quality writing) Flashrom type sanded off under the AMD logo.
The other choices now are try a hotswap or flash it in another box with Flashbios 3.03 bank. If both these options produce a failed flash then return the chip for replacement

You seem to have gone through everything correctly and you have more than tried all ways to get it to work as it should. If you have another chip i would fit that but you certainly want  the non flashable one you have now exchanging by the supplier.

Hmm.. I'm willing to bet you have a bad solder point on LPC that's letting the chip load a bios (flashbios, cromwell), but not letting you flash over it... Check your soldering nd try again.. If that fails then by all means exchange the chip.. it just seems odd to me that a chip without flash protection would load a bios but not be able to be reflashable...

Chip looks genuine. definiteley go with the resolder as advised by appleguru. You need to exhaust all possibilities first.
