you'll need to flash it as cromwell is a linux bios, in a 1.0-1.5 box i would recommend an Xecuter2 bios

The bios to use depends on the box version. So if you have a 1.6 make sure to get the correct evox M8.16 or M8+.16
Previous versions you can use M8+ or an xecuter bios 4981, 4983 etc

Yes, its fine to flash Bank 0

Make sure you download the evtool for the M8 Bios.
http://dwl.xbox-scen...Vtool_1.0.9.rar

when you have it simply open it, open the M8 bios and hit size.  Make sure you have 512KB Selected and it will ask you what to save it as.

You cannot boot from bank 0 and flash with just a bios disc. You need Eurasia to  flash if using the flash bios on bank 0 as it is Linux Based. If you want to flash using only a bios disc you must boot from bank 1 which is Flashbios 3.03.
Also the bank you boot from does not have to be the bank you flash, you can flick the switch just before the flash.

For a start you dont have to use bank 0

Heres what to do

Change the bank to bank1 flash and yeah thats it, on bank1 u can flash with just a disk (bank1 is second bank on the chip)

Switch banks like they said...  If your disc still doesn't work you can try this like Lepper said...

If you see the Evox shield in the top left you know it worked, or you could try burning a dashboard to a cdrw and renaming the .xbe file to default.xbe and booting that.  Seriously though, you have to do some reading/searching on your own.  You can't expect everyone to hold you hand through every step of the modding process.  There are lots of good tutorials out there, start reading them.
