You only tried one brand. Get some others like Memorex or something.

An old Verbatum I had lying around worked the first time I wrote to it. I had never opened it. But when I rewrote to it it would not load. So I guess make sure you try a fresh disc.

I got a DVD+R to work yesterday.

Slayers has a lot of useless junk but it has options for install so just use it to install the evox and then FTP the rest of your installs.

I just used XBMC, don't see the need for anything else.

Dummy files are a Dreamcast holdover that have NO VALUE on the Xbox.

This is more of a problem with your writer or recording software than anything else, assuming you are using DVD-R media.
