i had the same problem with mine when installing it, if its the pogo pin version, it can get slightly nocked out of place when screwing the lid on, try it before the lids screwed in

and when screwing it on, be very careful with the screw nearest to the chip

Pogopin installation may get loose contact on one or more pins for various reasons (make a forum search if you want to know more). "Flashing red and green" usually means that D0 signal is ok and therefore Xbox tries to boot external bios (mod bios), but the loading fails. This can be caused by e.g. that one of the LPC pins has lost contact or the mod bios can be corrupted/blank.
