The numbers in there refer to the BIOS number.
The 6 comuns are 6 different ways to partition up your  2M worth of BIOS.
If you use column 1, you only get ONE BIOS.  (And it has to be 2M!  So if you burn a 256K bios, you have to pad it out with an extra 1.75M worth of space!)
In this case, the Dip switches would be set to 1, or off off off.

In Column 2, you've got the option to select between 2 1M bioses, located at 1, or 2. (OFF OFF OFF, ON OFF OFF.)  Still not that useful.  Keep in mind that if you pick BIOS 4 or 5... there _is_ no BIOS with that number, so the thing's going to FRAG.

Move over to Column 4, and now you've got the ability to have a little of everything-  2 256K bioses, 1 512K bios, and a full 1M bios.  (numbered 1-4.)

Finally, in column 6, you've got 6 different bioses to choose from.  What had happened was that you had put yours in BIOS 5, but you were selecting BIOS 6, with ON OFF ON.  If you had changed the dips to off ON off, it would have worked too.
