odds are your default.xbe file is corrupt. Try re-downloading the dashboard from wherever you got it. If it wasn't Xbins, get it from there.

It was from xbins, i've also tried 4 previous UnleashX versions, they all exhibit the same problem.

are you using a softmod or a chip?

It's a chip.

I can't remember which one it is, but if it helps when it boots in the top left hand corner there's a pink logo with 'evox' written in it (evox chip?).

OK, that means you've got exactly the same setup as me.
I can't think of anything that would be a problem. I've looked at mine and from what i can see you aren't missing any files.
Maybe just delete the whole thing and re-FTP it to your xbox.
