The most reliable way is to put the original game in and rip it using the 360.

Um, the question was "iso" not "original physical copy of a pressed game disc". Even using burned DVD-DL isn't an option for some because it's egregiously expensive.

I use extract-xiso, I've been having a hard time finding anything else that actually succeeds constantly. I don't like ISO2GOD because it creates oversized packages. I prefer the extract-and-FTP method.

using the latest iso2god you can do a full padding removal and remove everything but the game to get it down to size.  freestyle dash does a great job a launching god games.

And what filesystem you use on the external usb drive? FatX? Fat32? From my understanding current impelmentations of filesystem access prevent fatx write, and fat32 has a 4gb file size limit
