You are positive an Opus and not a Xenon ?  Give out the manufacture date.  If 2005 or 2006 - its a Xenon.

Sounds like there's a problem with either the burns or media.

Failing to install means it couldnt' read everything 100% just because you played it OK it may just be you didn't need to access those parts of disc with where you were up to, sooner or later you may have encountered same error whilst playing.

one of my boxes now refuses to load or install just about anything (brand new originals included) for a while it would do what you describe. MS just use shitty drives and then compound the issues they have by all their anti piracy crap making things work outside their intended original purpose.
