This is my first time playing the elder scroll series and I dont have the game manual, can some one please explain to me what the four icons (the two right ones are a heart and a dagger, I can make out the left two) represent when you are on the weapons page in your journal?

Also how can I check to see if my weapons/armor is damaged?
