I recently softmodded a friends Xbox using xboxhdm 1.9 and UDE 1.4. The bios I chose was the inlcuded Evox M8 version. Normal IGR works (L+R+Back+Start) even when IGR is turned off in the Evox menu. That isn't a problem though.
The problem is that an extra method of IGR is somehow enabled. If L+R are pressed as well as the two thumsticks pushed down the game will reset. This becomes a problem in games like Halo 2 where you dual-wield guns and hold L+R to fire them and press one thumbstick to melee/snipe. If the other thumbstick is accidently pressed down it resets.
I checked the evox.ini file and all instances of IGR are set to NO and yet it still resets when the thumbsticks are pressed down. Any ideas? If the setup info helps it's below.

That is odd.. Aside from being turned off in evox.ini, are the igr keys in the ini set to what is triggering? Might be bugging out.. Evox igr is separate from bios igr.. If your bios supports igr there is no need for evox's settings as you noticed.. Can't hurt to try a different bios or nkpatcher..
Also, try setting Evox TSR to No..

I use the m8 bios also (1.4) and don't have igr with the thumbsticks. only the back and start L R combo works. Setting the evox.ini igr to no thakes no effect since the igr is in the bios and not in the dash.

i was having the exact same problem... rebooting constantly in certain fps games. turns out.. i had an older version of the m8 bios that includes the trigger IGR function (read the nfo on evtool. this feature was changed in 1.05 i believe). here's what i did to fix.
download metoo-m8pack-v1.4.zip from the usual places.
use the evtool that comes with it to open your old backed up bios from e\BiosLD\xboxrom.bin (that's what it is for me anyway) and notice the path to your dashboard.
open the NEW bios that you just downloaded (whichever one you want in the zip) and change the dashboard path to match your old one.
save this file as the same name.
then, transfer it to your xbox overwriting the old one in e:\biosLD
reboot.
works great for me, hope that was easy enough to understand

Thanks unipooper, the new bios worked perfectly. I guess UDE 1.4 just had an older version.
