Sorry to tell you this but the solderless adapter is probably bad. I just had the same thing happen about three weeks ago and I had to order another one from Divineo. I installed the chip within 5 minutes of getting the new adapter and everything has been ok since. I also had the USA (black universal solderless adapter).
This has been the only bad USA out of 10 installs that I have performed for family.
