You HAVE to connect D0 to a ground. It basically forces the motherboard to boot from the LPC (the Chameleon is a LPC mod chip). The Chameleon itself allows you to connect point D0 to a point on the Chameleon which will also work as a ground.
The reason your mod chip did not boot in the beginning is because D0 was not grounded. Once it's grounded, you may have a short or incorrect connection between LPC connections (red/green flashing light). I would check those points next with a multimeter.

The Chameleon is supposed to control the grounding of D0, so connecting it to the D0 point on the Chameleon should be as good as manually grounding it. The reason I suggest manually grounding D0 for testing is to find out if the Chameleon is controlling (grounding) D0 correctly, or if you have connected the wrong point on the motherboard.
If you are grounding D0 manually, and your Xbox FRAGs, that usually means your LPC points are bad, or you have no BIOS on your chip. Unfortunately, it could also mean your chip is faulty.

Here's my current status: When I manually ground D0, the box reboots twice and then (on the third cycle) flashes red and green. If I don't ground D0, the box boots the MS blob.
I've got three chameleon's here, they all behave the same, so I doubt the chip is bad. They might be blank, though...Any sure way to tell?
Could my A15 be bad? How would I tell?
And, I guess I should go resolder my LPC pins, too, huh?
Should the LED light up? Right now, it stays dark. Last night, it would blink green/blue, then go dark.

Recheck your LPC points, but first make 100% sure you have attached the D0 wire to the right point on the motherboard. If you have attached it to the wrong point, you could also get the FRAG.

Yes D0 MUST be grounded.
Make sure your D0 connection is clean. Check surrounding points with a multimeter as always. Make sure you have the correct point, it shouldn't matter whether you're using the Chameleon's ground point or a ground point in the motherboard, as long as you have the correct D0 and it's grounded.
Next move to the LPC points. Check every combination of points nearby with a multimeter. Often there's a short between points (no matter what type of installation you used). I recommend doing the pin header installation, make sure ALL points are clean and are not shorted. I've installed TONS of XBOX mod chips, and I have to admit that I have had points that I didn't think were shorted until I checked with a multimeter. (Which brings up a word of advice, ALWAYS check for shorts with a multimeter before turning the XBOX on).
The problem with a lot of newbie installers is that they blame everything on the chip. About 90% of the time it's because the installer was inexperienced.
