Check your IDE and power connections to the DVD drive and hard drive, make sure they are firmly fastened. Otherwise it sounds as though your hard drive is broken, so you'll need to replace it. As your Xbox is sofmodded you'll have to use xboxhdm and your eeprom backup - a quick search will throw up loads of tutorials and posts.
