Have you tested your backups with kprobe or Nero speed? Even though the media is good, the burn quality could be bad. I used MKM001's as well and achieved the best results burning at 4x (score of 95 on NeroSpeed) with the following settings.
EEPROM: enable all except for OHT (disable) clear OPC (also make sure to power cycle drive to make changes effective)
IMGBURN: OPC off, burnproof off

I would say this is due to a weak laser, XGD3 backups are known to be harder to read than XGD2 and originals. You could try a pot tweak or install a new laser. I would go for the pot tweak first, solved my problems in the past!
