you can also try XBCreator (and can change the read speed in the options.)

No, not that I know of

Since your problem was the extraction and if there is no scratch (and it is not dirty)  it should extract without errors.
Trying to extract it again will not cost you any DVD, if you  still have extract errors then do not burn it.

Have you done a few backups with this setup before?

if yes, as mentioned you can try with XBCr (and try different read speed) or extract each pieces separately(game.iso, ss, dmi, pfi) and then merge it.
 and if it does not work give a look at this post.

I was able to backup my copy of the Godfather using the following setup:

Ripped using MS-28 drive and XBC 2.3 and burned using Pioneer 111D and XBC 2.3.
