1) Read the tutorials that are out there, make sure you follow them to the T. If you are still having problems after that, try these things.
2) If you are getting port 170, a0, drive not detected, or status=00, make sure you have tried the instructions for editing your mtkflash for your chipset before trying anything else. I know they are in Textbook's tutorial and maybe other ones as well. These codes usually appear when mtkflash can't find the right port for your SATA interface or card.
3) If you have tried hex editing mtkflash (as indicated by item 2) and you are still getting drive not detected, status=72, status=51, status=80 or only occasionally is your drive detected but never reads or flashes, make sure you change your SATA cable. Also if you are using a Xeno or Xecuter Blaster, remove it and go with the good old fashioned croc clip and direct SATA link to the xbox 360 drive. This ended up being the main problem for me. Go figure, I buy a Xeno so I won't have to take appart my xbox to flash the drive and IT ended up being the problem.
4) Make sure your VT6421 SATA card is seated in the PCI slot properly.
5) Take all of the other PCI cards (the ones you can) out of your system and try the VT6421 card in every slot.
6) Try different SATA ports on the VT6421 card.
7) Disable all other drives in your system (where possible).
8) Try different timings, even with the problem I was having through the Xeno, the drive detected some of the time.
9) Post on the forums and see if someone can help, be sure to include your drive type, the chipset of the SATA port on your motherboard (or card, if you aren't using your motherboard's SATA), and the things you have tried before.
10) Don't give up and keep reading and trying different things.
The problems I had with my drive kept me trying unsuccessfully to flash it for well over 7 days. And then after all the reading, researching, trial and error, timing and counting, and pulling my hair out it turned out to be as simple as not using my Xeno between my computer and my xbox drive.
Hope this helps at least one person out there,

So look out for bad sata cards or cables.

Thanks to your troubleshooting Guide, I will try all the things you mentioned.
I have almost identical setup Ms28, with Via 6421L. Using Iprep w/ 1.2 xtreme bios.

I've been trying to flash my Ms28 for 2 weeks now and everything I try, just don't work. But there's some things I believe you guys can help me.
In Iprep, there's a space call "Dev id:" and "Dev: iO", What is this? Where do I find this codes put on?
My mobo is a Asus K8V-VM ULTRA and my chipset is a Via K8M890ce, but unfortunately it doesn't recognize my drive for SH**.
I need a help from you guys who have already done this.

That mobo should work as it is a VIA 8237 chipset...
Are you selecting the chipset in iPrep?? The device and IO should fill in automatically..

I am trying to flash my ms28 drive with xtreme 4.2A already loaded. I have read many tut's, but cannot seem to flash my drive.
I am using a Via 6421 sata controller card, tried all slots on my mobo.
When I launch mtkflash upon boot, it will detect the drive as an xtreme Pri Master. But I get the following line after I select that drive "Port: ec00, Master/Slave: a0", then it just hangs. Tried counting to ten after i select the drive, but still nothing.
any suggestions?

You need to turn off your xbox before you select your xtreme Pri Master, as soon as you select the xtreme Pri Master start counting, once you reach 10 power on your 360 with the Eject button, then it should start flashing.
