how are you powering the drive ?? Through the 360 ??
Check all connections.
Check your mtkflash (i.e. use hexworkshop)

when you boot up via the usb or floppy, what exactly do you see when you type the mtkflash command?  i have a couple 6421A cards and what i viewed on screen was pri master, sec master and then xtreme as options, not the card like i viewed in the videos.  i selected the xtreme option, did the 10 sec trick (have ms28) and it all worked (IMG:style_emoticons/default/smile.gif)

i remember someone talking about a jumper setting, the stock way didn't work, doubt its on very many cards but its worth a look

FYI: only use the sata slot closes to the front of the computer
