i didnt have this happen, and I have live...got it after getting rainbow six 3....
i think yours is an exception...maybe shanafan can chime in on this.

I started up the game, and it told me to create a profile. I created "shanbox" (my gamertag is shanafan).
Game loaded to main menu, no connect to Live.
This again confirms my findings.

I could sign in Live, navigate fine, and then sign out.

maybe he hit the x button at the main menu? hard to tell what people actually do and dont say.

I have no idea why this happened - it tries to connect using my gamertag after I create a profile for the first time even if I give the profile a different name.
If I ever get back on live this is something to learn from...don't take it for granted that using a different profile name to your gamertag will always mean that a live aware game won't automatically connect.
I'm sure for some people this never happens but there is always the exception.

How can there be an exception? Everybody's Xbox is the same. The game either doesn't connect to Live, or it does.

I disabled my Chameleon and went into the MS dash. Enabled auto sign-in. I turned XBox off, and then back. Disabled Chameleon, and went into MS dash again. Live loaded as expected.
Loaded Rainbow Six 3, and it did connect to Live with my gamertag, even though I did not have it created as a profile in the game.
So, the original poster has auto sign-in enabled. Which should be off, when following my Live tutorials
Nice heads up anyways.
