Frustrated With Error 16 Issue

I had a friend mod my X-Box.  He soddered a SmartXX LT OPX chip.

The X-Box starts up just fine about 40% of the time.
The other times, it acts like it's turning on and it turns off, it turns on and then back off, turns back on and then goes right to the error screen with error code 16.

What is the problem?  Bad soddering job?

Yes. 3-boot-then-frag is almost always a bad soldering job. Check the solder points, especially the d0. Most likely one of them was poorly soldered and is making intermittent contact, resulting in the intermittent booting.
