Error 16 After Trying To Fix Softmod

I would run Atapwd.exe and make sure the drive is unlocked.

Try formatting the drive with a 98 boot disk. You can make one from www.bootdisk.com. After you Fdisk it and format it, retry building it with Xboxhdm. You can only do this if you have the eeprom.bin backed up.

Install a modchip (temp) and run slayers ?

Run Slayers to FTP in and get backup or back it up with slayers. Do not install slayers, as the stock bios will not boot to evoxdash.xbe as default. TSOP flash is the simple way to go..... Then you can flash and install Slayers.
