1.6b Sp Ice Install, Led Won't Turn Off

Hello all, thanks in advance for any help.

My problem is this:  I have installed a xenium spice on a 1.6b.  I get all 5 leds on the adapter to turn on.  I then place the modchip on the adapter, and the led that is supposed to turn off does not.  I believe that the led turning off is indicative of the modchip being active?  The led on the modchip itself turns on, but that second to last led on the adapter won't turn off.  I have checked to make sure that the spring loaded pins on the adapter are properly seated, it seems so.  I have removed and reinstalled the chip a few dozen times hoping for a different outcome, no luck.  I have also installed the additional rivots into the holes that the spring loaded pins from the adapter fit.  The xbox boots the MS bios just fine.   Any suggestions?
