Returns On Potential Bad Chips

Hello All, I just purchased the Xenium SPICE (S.P.ICE) mod chip from Extreme-Mods.com. I bought the $60 one that supports all 6 versions because I purchased it before I purchased the X-box and did not know what version I would end up with.
Well here's the deal, I installed the chip exactly per the "Iceguide" and when I first turned on the X-box after about 8 seconds or so I saw a puff of smoke from the chip.  It did not smell bad like it usually would but I definately saw a puff of smoke.. I do not see any burn marks on the board anyware.  

So what happens when I boot is this sequence:
turn on got green light on the front button
then it turns off (by itself)
then it turns back on (by itself) and the front light goes green for a sec then starts fragging

The light on the mod chip is blue all the time

Has anyone seen this issue before or have any idea what I might do..   I really need to assume the chip is bad but you guys might tell me different.

If I tell Extreme mods about the puff of smoke will that help or hinder my chances of them replacing it for free?
