Check your recovery switch on the chip - make sure the black dot is covered (if I remember right) or you could possibly have a bad solderless adapter.  I have modded 6 Xboxs with Xenium Ice and universal solderless adapters and have never had a problem until this last install I attempted.  I have the same problem as you and after some very extensive searching, a lot of banging my head against the wall and trial and error I have come to the conclusion that it is the solderless adapter.  I ordered another one today from Divineo.  Hope this helps.
