You should really update your OS and error 12 is a dvd error I believe.  Check and make sure all connections are tight on the DVD.  What version is your Xbox?  If its not a 1.6 don't use M8plus16 as I think that that is only for ver 1.6 boxes.  I could be wrong though.  I repair all of my problems throgh trial and error and a LOT of research.  90% of my research is from this forum.  I'm sure that there is a thread somewhere about this problem just search for it and you will probably find your answer quicker than posting.
