at one time i had a weird controller issue, i just truned off the system think it worked... is that your only controller?  did you try any others?  is this occuring with like one button like lets say a for def jam is grab and a for 007 is shoot are those having problems?  because it might just be a jammed button or what not.  try clearing out your cache's iny our e folders,  go to E:\Cache and delete all the files within it.
