If you can, swap out a PC cdrom to get the chip flashed.
This should bypass most read problems.

Make sure you are using a known good bios from a reputable souce like #xbins
Check the MD5 packaged with the file to ensure a golden image.
Read the NFO.
Here is one such MD5 checking application.

If you are rolling your own 256k BIOS make sure you are using the 4th/last segment.

I would really reccomend trying a PC DVD drive if you can.  I have a BIOS cd that I used in several XBox (Thompson, Samsung, etc) and worked fine until I get to one xbox of a friends.  Kept saying BIOS wasn't sized properly although I knew I had a good cd.

I tried my Samsung drive in his XBox as I knew that could read the CD as well but I got the same error.  The solution was to temporailly hook up a PC DVD drive just for the BIOS flashing...worked propelry.

as it turns out... im a moron

i have a maxtor 120 harddrive with a bunch of different BIOS files on it - all from slayers. (in my other xbox)

so earlier i had tried to select the bios from the harddrive and it said "bank not yet selected" so i selected the bank and it said "file not yet selected" and i never tried selecting both one after the other before going ahead with the flash...

anyways, it went smoother than i thought it ever would, and i flashed bank 4 with x2 4977... now im exactly where i wanna be, without having to burn any more TDK's...

im a happy camper again
