Check your soldering.  Especially on the 1.0 as I can almost be certain your problem is your soldering.

v1.0- I agree, it seems that the problem is with the soldering.  if you want to make sure, use the DuoX2 chip that was a success.

other xbox = try flashing with the dvd and HD plugged in the mobo.  I had that problem once, when i was trying to flash a new DuoX2 without a HD attached.  I was finally able to flash when I turned it off and attached both the dvd and HD.
