You tried using rivets??? for what.
1 These chips all come flashed.
2 This is a soldered in chip. How the heck are you trying to fit it?
3 what do you mean you  took out the D0 pin
I don't think you have done something wrong, it sounds like you have done evrything wrong.
You need to have another look at the install instructions on the Duo site and follow them or at the minimum give detailed info here on how you are trying to install this.. I get the distinct feeling you are trying to use a solderless adapter from another chip

I have heard word of a solderless version coming but not from them directly so put it down to rumour myself...

the above link shows no mention of solderless adaptors!

BTW - the problem you describe is exactly why I would never use one - they are not to be trusted - i.e. not reliable - bump or drop your station and off comes the adaptor - or worse it becomes missaligned and blows your station on power up.
