Check you contacts.
It could bethat the solder-less adapter was not installed snug enough the firat time round.
If you have a Xbox version with the LPC holes not filled than make sure that you are using the rivets. These are required for those Xbox version. make sure that no pogo-pins are stuck in the adapter and make sure that the D0 is making proper contact with the gold wire.
It sounds like a clasic contact issue problem to me.
