What type controller is it?  Do you have a DVD kit installed on your xbox?

MS brand xbox controllers, yes i do have a dvd kit installed tried it with it plugged in and without.

This is only a problem with the Cromwell BIOS, or is it a problem when you boot other BIOS's as well?

Have you tried the EvoX XCHIP BIOS ver 1.0 ?

Cromwell has been known to have some issues with some controllers before.
