What are the dip switche settings currently.
How are you aligning the chip
Have you tried cleaning the d0 and lpc

I set them to write mode (1off 2off) to write and then to 1 off 2 on when in the XBOX. The light on the chip is solid green when powered on. I align the chip manually and then plug it in prior to re-installing the HDD & DVD drives.

I have not cleaned anything yet.

I cleaned the d0 and the LPC. I also cleaned the pogo-pins themselves. I used a wire wheel on a dremel at a very low speed. The points are much shinier but I still have the same results.

Any more ideas?

I fixed it. I had the screw in the chip too tight. Apparently the green LED on the chip isn't always a sure fire sign that you have the chip installed correctly. After moving the chip around a few times I started getting the XECUTER2 logo during bootup.
