I take it your using a matrix.  Did you remember to switch the matrix from mode 1 to mode 2, 3, or 4?  Mode 1 disables the mod and is used for aligning.

My guess is that it has come unaligned again. Re-align and try again.

If he installed the chip in mode 1 to align it, he would have had to put it into mode 2 to FTP to it and read the disc. How would flash red, since it always stay green in mode 2...

Maybe mode 3?

if the console is a v1.1, fill the LPC holes with solder.
Then, (incl if it is a v1.0 console), clean the LPC points & D0 point with alcohol or similar cleaning agent. Try using a slightly longer screw too, making sure the mod is secure.
