Have you checked to see if your modchip is seated and soldered correctly on your Xbox's motherboard?

I would reflow all the solder points if everything looks fine just to make sure you don't have a dry joint.

Post some pics if you can.  that way we can see if anything is out of place, etc.  You could also try another chip if you have access to one.  But the way it started flaking on you over time makes me think it is related to the solder.
