You checked all the settings in-game? Tried a diffrent controller? if so it sounds like its the little bit that plugs in.. try opening it up and seeing if theres anything noticible.
Oh iv heard of the volume controller getting stuck or braking open it up and have a look at that.. Replacement headsets are like £3 from gamestation in the uk.
