If an illegal gets injured the hospital is required to give them adequate treatment even if they cant pay.  The hospital eats some of it (actually they just inflate the costs of their services for the rest of us to cover it), and Health and Welfare re-imburses for a part of it.  Either way the public really ends up paying for it.  This is a really big problem in parts of the valley I live in because most of the violent crimes are commited by illegals against other illegals which often results in very expensive surgical procedures to save their lives.  Anyways, thats how illegals get free health care and thats one way they leach.  If they pay anything at all, its just the money they had in their wallet.

Also,  a lot of states like mine and california are required to educate illegals in our school systems which they dont a pay a dime into.  Thats a second way they leach

there are probably more ways, but those are the two biggies in my state.
