You need to replace it with the original type of drive.  Samsung drives can't do AP25, so that is probably what your problem is.  You probably had a drive that can do AP25 and your system is expecting to be able to do those challenges and when it cannot, it will give you an error.

From now on, disc drives need to be replaced with same model.

If its not done, it will give you trouble somewhere down the road.

Ahh my box has a hitachi 49dj in it. but it stoped reading backups.

so i stuck a samsung ms28 spoofed as the hitachi poping in my gears 3 original and got the e66..

If you're using X360USB Pro go to page 194 of latest JungleFlasher tutorial (1.4.4) - Bad Flash Recovery.
If you have VIA 6421a chipset you can try using bad flash recovery method in DOS (iPrep) - google 'x360 Textbook' for tutorial.
DOS method has always worked for me - I've only recently got my X360USB Pro and haven't needed to try bad flash recovery with that as yet.
