Hummmm strange but I have the same thing ....
I've installed Afro Samurai first on 120Gb Official HDD (in fact not really : it's a Western Digital inside ...) ! The game was working great but launching my savegame make it bug (unreadable disc message, funny from hard drive). After that I've changed my mind, and choose to have the game on the USB HDD (that is a 500gb Western Digital). So I convert it from GOD to XeX format ... copy it on the USB HDD : same problem.
So i've tried other things : installing the game directly with the Xbox360 drive, to the USB HDD drive : same thing ! Etc ... etc ... etc ...
I wasn't really aware about patching XeX files, so I was asking myself : maybe it need to be patched ?
So i've used another USB HDD (in fact the Samsung 20Gb Xbox360 official that was in place before the 120Gb one ... but i've formatted it to FAT32...) to copy the game and try to patch the XEX files ! And how interresting : launching my savegame works now ! So I've tried again, with the same HDD and the original XeX file : works fine too !
It was really strange ... so I decide to check my 500Gb USB HDD ... Seven found me problems and solved them ... trying again the game from it : same problem again !
Well ...
Now I have Afro Samurai on my 500Gb USB HDD working fine ! (I've finished the game using my savegame (IMG:

As everybody knows, we can't format 500Gb HDD to FAT32 format using Windows ... So I've found and used a freeware named FAT32Formatter that make it ! But the drive was really really slow to be recogniez on Windows ! And make my JTag Xbox360 boot even longer ! (Only the first time when I've turned it on !)
Windows found me little problems on my (brand new) 500Gb HDD, so I've decided to download maintenance software from Western Digital to check my drive in a better way ! And so I have been surprised to found on the wesite page of my drive a little software named Western Digital FAT32 formatter (EXTFAT32.EXE) Version 2.0 ....
So I decide to format again my HDD, backup data first ... then copy it back and defrag it all ... after that the drive was really faster to be recognize on Windows ! My JTag Xbox360 boot wasn't that longer too ! And my savegame of Afro Samurai was working !
So maybe this is the way to see ... how do you have format (everybody) your USB HDD ? And I don't want to be bad but how about the manufacturer ? For example Western Digital and Samsung (or Hitachi etc) isn't the same quality at all ...
