Delete it off your HDD, but only delete the profile, not the profile and items. Then recover it from Xbox LIVE. Fixes profiles problems almost 100% of the time.

It's been stated before that removing homebrew achievements will cause problems on profiles that you wish to use on Live.

There is no other way than to retrieve your profile from Live and regain those achievements.

As a side note, I know of a few JTAG users that have actually sync'd with Live after gaining homebrew achievements and they have had 0 issues. However, it's extremely obvious to Microsoft, and in the future they could ban you easily.

I have a similar issue but don't have access to a NON-jtag console at this time.
I've ran my profile thru Le Fluffie (everything shows true) and Avatar items have been ran thru Yaris Swap and rehashed/resigned and done all the XUID stuff and all LOOKS good but on some games like NFS:HP it asks me to make an avatar even though I clearly have one when signing in.
When I exit the game I have no avatar or gamerscore showing. Also, if I try and make a new avatar I get the "can't save your avatar. make sure your storage device is plugged in" notice.
I've already applied the official 12611 update via usb which gave me back the avatars.
It's a Live profile and has snes achievements but I don't care to use it on Live, just want it to work the way it should.

Edit: Can I rebuild my profile with Le Fluffie to possibly fix this as an alternative to the Live route, it seems to have that option?

Well to answer my own question and for anyone else with this issue I did a clean rebuild of my live profile with only my 'Account' file included and all the necessary details.
After transferring it over and signing in I was asked to create an avatar but still get the 'can't save your avatar' BS so at this stage recovering the profile off Live seems the only choice which I would have done by now if I had access to a non-modded console.
I haven't been on Live for ages so I'm hoping to add the games and achievements I've earned genuinely after that.

Hey boss hope this helps. If you still have save files. Recover the most recent gametag version from live then insert game missing achievements then load up your save. Any achievements you earned in the save file should just repop basically ciao
