Rebuilding Iso's

i'm trying to rebuild a japanese imported iso i just bought so that i can play it on a usa 360 console. i have a benq flashed to 1.6. i assumed it could play all retails as long as it was ntsc. but i was wrong as i found out when i popped it in. so after some research, i learned that japan games are region locked. however, i did find a tool that can make the disc region free by changing the default.xex file.  now the problem i am having is re-building the iso to a proper 360 image once that file is changed. i've tried xdvdmueller beta 10 to do  it, but i keep getting errors. what is the proper file structure to create or rebuild a new iso?

i tried changing the default.xex file to region free with xextool.  everything checked okay through abgdx, but when i put the disc in, it said unreadable, disc could be dirty. it didn't say wrong region or anything like that. just unrecognizable. maybe the iso wasn't built correctly. i don't see how that would be. when looking at the image, it has the same contents as any other ripped backup. this is really puzzling me.
