Also what media did you use and what firmware revision do you have on your 360 DVD-ROM (not to mention which DVD-ROM drive you have in your 360).

I had the same thing happen to me. Sometimes my backups only run when the xbox is cold.  Try the rubber band trick.

this happens to me as well after the fall update .. some backups working 100% before now only work the first time .. this with hitachi 47dj garyopa 2.3.. the first time runs perfectly ...but if i turn off the console or change game and go back the game starts but when appear the intro .. unreadble disc..and no go.. very strange.. i tried re-flash the drive with no luck.
