What Is Wrong With My Backups?

I've tried backing up Oblivion but everytime I try to play it, I get the dirty disc error.

I'm using the VIA6421 chipset with the pioneer-dvr111d dvd burner and verbatim media. (CloneCD allows me to burn it at 6x). My xbox 360 has a samsung drive with firmware MS=25.

So I flash my drive using the xtrm0800.bin firmware and restart my computer so that my computer could detect the drive.  Use DVDInfoPro and send the custom commnads AD 00 FF 02 FD FF FE 00 08 00 01 C0 for the first, and do it chaging the 10th one to 03, 05 then 07 making it 4 times and save the .bin to get the SS.

in the preset commands tab, i switch to Media, and then send out FF 08 01 01 00 00 00 00 00 00. After I send it out, a get some message.
"Command 0ffh returned.
Sense 00h           No Sense
ASC 00h
ASCQ 00h            No more Info"

I see a sense error occured in the LED part (bottom left corner of dvdinfopro).  But I am able to open IsoBuster and I see the one track instead of the VIDEO_TS and AUDIO_TS folders.

I right-click the CD and choose "Extract from-to".  Change the length to 3567872, and set extraction type to User Data (2048 bytes/block for MOST sectors) and I start the extraction.  It prompts me to save the file and I can only save in the .tao extension, but I save it anyways.

After the extraction, I change the SS file name to ss.bin and I change the image Game.tao to Game.iso.  I start build360.bat and let it run.  I change the iso file name to IMAGE.000 and make sure the IMAGE.DVD file is pointing to it.  I then burn on CloneCD at 2.4x

Reflash to my modified firmware, start the 360, put in the game and I get the dirty disc error.

Can I get some help to resolve this problem?

your drive is already unlocked at that point... get the SS, dont do the last command, just open isobuster.

edit: also, dont know if it matters, but SAVE AS game.iso, dont make it a tao and just change the extension, that may not work

using the extract from-to i cannot change the extension
