Help With Making A Component Cable

Being an old audiophile, I've done my share of tweaking and soldering on audio equipment. This is a little different for me and would like to make sure I don't make any mistakes. I will be using an extra Monster Cable component video cable that I have - chopping off one end for soldering.

I've printed the pinout diagram for the A/V connection and have the following questions:

1) What is the connector called and is it readily available at a Radio Shack type store or will it have to be ordered online from a computer supplier?
2) The 2nd page speaks of a "setting cable ID" What is this for?

I plan on simply soldering the Y, Cb, and Cr cables to the appropriate pins and using a optical digital cable for the audio. Isn't it that simple, or am I overlooking something?
