I would try shorting pins 44-47 on the nand. Has worked for me on several ocassions. Had a falcon that kept messing up.

If you put the front of the xbox towards yourself 48 is on the top left. Unplug the xbox, short pins, plug in xbox, then remove the short. Then try to read the nand. May take a few trys but it should work.

Also check the wires on the usb reader. I bought one off ebay from somebody here and they were fine for a while but they started to break off. Good reader but his choice on a wire connector was terrible.
