Hey pal I had the exact same problem the only way I got it to work was with a bootable floppy. I know the floppy method is risky so I dumped the firmaware twice on 2 different boot disks to make sure.
The only reason I can think this is happening is that there is some trouble writing to the usb drive.

thanks, that's a good idea, of course i was using the oldest crappiest usb stick i could scrounge from the depths of my wifes purse  (IMG:style_emoticons/default/ph34r.gif) (she didn't even know it was buried in there), it was writing a 0 byte orig.bin - won't be able to try again till tonite, but i will def try a diff usb first, i've still got a couple floppy drives but i might have too dig deep for a disk  (IMG:style_emoticons/default/smile.gif)

more ideas appreciated - i'll try em all
