disconnect xbox till floppy boots to a:\  then plug sata in with xbox already on type mtk etc...
try that  (IMG:style_emoticons/default/uhh.gif)

Just flashed to Ixtreme.
I made myself a verify.bin to check everything matches up OK - anyone know how I verify my flash using a hex editor? I got the ix10.bin file i flashed with and the verify.bin I made afterwards, was wondering if there was some quick way I could verify both files match rather than going through every single line in a hex editor.

Out of interest, anyone know how c4eva writes these firmware files, I'd love to have a look at the code or something....he can't write it all in hex...

Well, i discussed it with c4e, and he double checked the public release and the fix for originals is definitly present. Likely the issue at hand is your previous firmware read originals at a slower speed (5.2D right?) and the new firmware is hardcoded at 12x. This could present a problem with worn-down lasers.

put them both in a ZIP or rar files and you will get the crc32 by selecting it as a column, that is sufficient.
if not I use HHD Hex editor that has a compare of two files, but a few others have it as well.

How would one upgrade from Xtreme42a? Do I need to restore back to the original firmware then flash it to IXtreme? Or do I have to upgrade to 52a or 53a?

Add /sata in to your command line. You should end up with:
mtkflash w /m /sata ix10.bin

Can you not downgrade firmware?  I installed iXtreme 1.0.  Found out I was banned, now I want to downgrade so I can play my backups that wouldn't work with iXtreme.  It's not letting me.  Is there a special command or something?

You need the 0800 disc to unlock firmguard.

Same thing was happening to me @ first. For me, it was timing. What got it to work when i doing my friends, was power on the console the moment it comes to dos, then eject the drive. I only let the disc spin for about 10secs, eject & it worked.
For mine, that didnt work & for w/e reason when i plugged my sata hdd back in & had the TS drive on the other channel, it picked it up. Just keep trying different methods is all i can say...unless someone knows better? Good luck!

make sure the 0800 disk is burned to a dual layer dvd, single layer dvds are no longer supported.
