Maximus 360 Firmware Toolbox - Post Here- Working Sata Chipsets

There seems to be plenty of confusion over what combination of SATA/connectivity kit/drive firmware is currently working with the Maximus 360 tool.
So - if you've got it working 100% can you post your -

SATA chipset (SIL, VIA etc etc)
SATA interface (On board, PCI, USB)
X360 drive version (v46, v78 etc)
Interface used for power (either from X360 or Connectivity kit)
Method used to get the drive in mode B (SLAX or Connectivity kit)
