Sounds to me like your laser is about to go. . . that, or your DVD media isn't ideal for your system. I've found that the lasers on the 360 drives are very finicky things. Fortunately, they're pretty cheap and easy to replace. You can pick them up for less than $10 on ebay.

Also, you can try doing a POT tweak to potentially squeeze a little more life out of your laser.

Try different brands of dual layer dvd.
