Dashboard Update Issue

recently i update 2 xboxes.  both have liteon 93450c and were on lt+1.9.  one updated to the latest dash without an issue, but the other got stuck with this status code (xxxx-xxxx-xxxx-xxxx-C000-0099 -  tampered device).

i have 2 questions regarding this...

1. is the flagging process random?
2. is there a way to remedy this?  (i already tried going back to original fw without success - i did the mra hack way back when myself)
