In my personal experience, every time I have read errors its because of wiring. Make sure all the wires going into your USB flasher are tight and making good contact, also keep the cable as short as possible.

USB doesn't matter about cable length (unless you've got a 3m+ cable). As SphtKr76's said, check your wiring, although if you're getting the correct flashconfig, this may not be the issue.

Don't do anything on the PC whilst your dumping - are you using the driver for the USB r/w that's in the NandPro folder? Also check that the USB r/w checks out in your Device Manager correctly.
