You need to rip the game, rip the ss.bin, then load up Ss Merger and 'Merge' the game and ss.bin together. Once this is done, you will have a Game.ISO that can be burned, aswell as a Game.DVD file which contains the LAYER BREAK information. You can either use CloneCD as the tutorials show, or DVD Decrypter.
Finally make sure your BOOKTYPE is set to DVD-ROM and not 'Normal'.
Basically do this checklist every time and you should rarely get a coaster:
1) I have verified my ISO and SS.bin (with SS Merger)
2) I have MERGED them together with SS Merger (after verifying)
3) I have specified the LAYER BREAK in my burning program
4) I have changed the BOOKTYPE of my DVD Burner to DVD-ROM.
If you miss one of these steps the game won't work. The ISO is the game itself, and the SS Bin is the 'serial number' in a sense. Just like Windows or any other app, If you don't have the serial number, you can't load the program. Same goes for Layer break and Booktype. The layer break is where layer 1 of your 'dual layer' disc stops and layer 2 begins. If this is incorrect, the 360 won't be able to read both layers properly.
Finally, the 'booktype' normally corresponds with whatever media you're using. For example, if you burn a DVD-R, the booktype is DVD-ROM. If you burn a DVD+R Dual Layer, the booktype is DVD+R DL. The 360 is programmed to only read games on DVD-ROM discs. So if you leave your Booktype to 'Normal', and burn your game to a DVD+R DL disc, the 360 will see that its a DL and not a DVD-ROM and won't boot it. If you change the booktype to DVD-ROM and then burn, the 360 will see it as a DVD-ROM disc even though it's really a DVD+R DL.
